In most patients who already have 2 testicles present but seek to enhance the size and appearance Dr Gallagher usually prefers a “side-by-side” technique. This means the native testicles are left in place and the newer larger testicle implants displace them out of view. This means that the native testicles can still be examined or accesses if needed.
To do this successfully the new testicle implants must be big enough to hide the native pair. The average testicle size in the US is about 3.5-5cm long and about 3cm wide. That is a volume of about 18-25cc.This roughly equites to 1.5 -2 inches long and about an inch wide.
Usually, the testicle size is maximum by the age of 20 and very gradually can decrease after the age of 50.
Seminiferous tubules which produce semen make up about 80% of the volume of the testicle so there is some evidence that decreased testicle size can be associated with decreased fertility.

Depending on the size of the native testicle and the patient’s goals a larger custom implant may be best to adequately displace the native testicles and meet the size requirements of the patient.
