What Is MTF Top Surgery?
Top surgery is performed to help transfeminine patients achieve a more rounded and sculpted chest profile that reflects their identity, boosts their confidence, and allows them to enjoy a wider range of clothing, lingerie, and swimwear options.
Many patients require a larger volume cup size. This can be done either using the patient’s own fat taken from other parts of the body where it is not wanted, usually the abdomen. This is known as fat grafting. It can also be done by using an implant. There are several options and approaches to consider. Dr. Gallagher will help you understand which option will be best to enhance and feminize the breast. For most patients, an implant is the more obvious choice, as with each fat grafting surgery, there is a limit to how much we can increase volume (usually it’s limited to only one cup size.
The main decisions you will make with Dr. Gallagher are:
- Type of implant — silicone vs. saline
- Placement of the incision
- Whether to place the implant above or below the muscle
- What size implant to use
Most patients opt for a silicone implant placed through an incision in the fold beneath the breast, beneath the muscle.
Size preference is very personal, and Dr. Gallagher will discuss the pros and cons in detail. Often, there is a “sweet spot” range that the chest can accommodate. If we go beyond this, we may introduce distortions that lead to unnatural results or droop over time. Therefore, Dr. Gallagher and the patient will typically agree on a range before the surgery.
