Introduction to MTF Top Surgery/Feminizing Mastoplasty

The surgery relies on breast implants or fat transfer to maximize the ongoing benefits of hormone therapy and help patients experience improved congruence between their inner identity and outer presentation. What Is MTF Top Surgery?

Top surgery is performed to help transfeminine patients achieve a more rounded and sculpted chest profile that reflects their identity, boosts their confidence, and allows them to enjoy a wider range of clothing, lingerie, and swimwear options.

Many patients require a larger volume cup size. This can be done either using the patient’s own fat taken from other parts of the body where it is not wanted, usually the abdomen. This is known as fat grafting. It can also be done by using an implant. There are several options and approaches to consider. Dr. Gallagher will help you understand which option will be best to enhance and feminize the breast. For most patients, an implant is the more obvious choice, as with each fat grafting surgery, there is a limit to how much we can increase volume (usually it’s limited to only one cup size.

The main decisions you will make with Dr. Gallagher are:

  • Type of implant — silicone vs. saline
  • Placement of the incision
  • Whether to place the implant above or below the muscle
  • What size implant to use

Most patients opt for a silicone implant placed through an incision in the fold beneath the breast, beneath the muscle.

Size preference is very personal, and Dr. Gallagher will discuss the pros and cons in detail. Often, there is a “sweet spot” range that the chest can accommodate. If we go beyond this, we may introduce distortions that lead to unnatural results or droop over time. Therefore, Dr. Gallagher and the patient will typically agree on a range before the surgery.

What Are the Benefits of MTF Top Surgery?

When transfeminine patients start hormones after puberty, it’s not unusual that the breasts may form a characteristic “tuberous” appearance. This usually indicates a lack of fullness and roundness in the lower breast. It is critical that your surgeon understands and has experience with the unique needs of a transfeminine breast to achieve the best possible results. When top surgery is performed by experienced, double-board-certified plastic surgeon Dr. Gallagher in Miami, patients enjoy the following transformative benefits:

  • Enhanced alignment between the inner and outer selves
  • Heightened confidence, well-being, and quality of life
  • A more feminine, balanced chest appearance
  • Expanded options for clothing, swimwear, and lingerie
  • Lasting, natural-looking results

MTF top surgery is suitable for healthy, non-smoking individuals who have realistic expectations and can devote the proper amount of time to recovery after surgery. It is often recommended that the patient be on feminizing hormones for 2 years to ascertain what breast growth they will naturally get before undergoing an augmentation. The WPATH Standards of Care mandate approval for patients with persistent, well-documented gender dysphoria and the full ability to consent to treatment.

What to Expect on the Day of MTF Top Surgery?

Dr. Gallagher will work to provide the most affirming result for each individual patient. There are often subtleties involved in breast augmentation for transfeminine patients that require attention and the adaptation of the technique in order to get the best results possible.

  • Growth Already There -First, we take into consideration what changes hormones have already caused. The typical advice is to wait 2 years at least, so we have had maximal benefit on the hormones and the breasts have stopped changing shape (we never want to operate on a moving target). We take into consideration the size and shape of the breasts.
  • Tuberous Breasts – Often, many transgender women may notice that they have tuberous-type breasts, which means the fold is high and the lower part of the breast is missing the rounded contour. Also, it’s very common to have asymmetrical breasts. Dr. Gallagher will take all these factors into consideration when choosing the size and position of the implants. Indeed, she will often choose two different-sized implants in order to balance out the size differences.

Transgender Woman Breast Surgery Options

Many transgender women may benefit from having the implant placed on top of the muscle. This prevents the implants from being pushed out of the sides and allows the rounded contour of the lower part of the implant to shape the lower part of the chest.

Often, the nipples on a transwoman’s chest are spaced a little further apart. While we want to create cleavage, this can be a little more challenging as it is critical to center the implant on the nipple. Typically, as swelling reduces and the implants settle, it is possible to get nice cleavage in a bra.

Mastopexy or Shaping

Sometimes, the breasts may require additional shaping or lifting to enhance their femininity. For example, in many transgender women, the fold underneath the breast may sit high. This can be adjusted. Patients typically undergo this procedure simultaneously with an augmentation.

Nipple Enlargement

Hormones can increase breast tissue, but a lot of transgender patients may notice disappointing feminization of the nipples themselves. Various surgical techniques can be used to increase the projection of the nipples.

I had an amazing experience with Dr.Gallagher and her team, the whole experience from the procedure (top surgery) to the post-op care has been fantastic. Dr.Gallagher was able to address any concerns I had with my chest before the procedure and made me feel comfortable the whole way through, and then the attentiveness post-op is all the better.

I had followed Dr. Gallagher for several years on Instagram prior to my surgery, and learned so much from her detailed explanations about various aspects of the surgical procedures, what to expect on the day, recovery, etc. I had complete trust, felt safe, felt listened to. Dr. Gallagher and her whole team, from Neasa to Daryl Ann and everyone else in between, are absolutely fantastic, kind hearted, hard working people. Dr. Gallagher truly amazed me and I’m so grateful to have had her on my team.

I was able to get keyhole top surgery about three weeks ago and I’m loving my results already! I’m muscular and heavier set as well but always had a smaller chest, and I’m so glad Dr. Gallagher was the one who could do this for me.

Right now I am still experiencing swelling, but I am using compression dressings every day, throughout the day which helps tons! I know that when the swelling subsides months from now, and everything is “back to normal”, I am going to love even more what I see in the mirror because I can finally see me.

So glad I found Dr. Gallagher. I would not have elected to pursue my surgery had I not found a qualified and competent medical provider that I felt I could trust and have confidence in their ability to offer a high-quality, low-risk result. As a result of my initial contacts and consultations with Gallagher Plastic Surgery practice, I was willing to pursue my desired surgery and even travel half-way across the country to their offices. I’m so glad I did. The results are outstanding, more than I even anticipated. This was the 3rd practice I had dealt with and so I was learning what to look for and knew Dr. Gallagher was a good fit. The medical expertise and professionalism are what I based my decision on to proceed. They were calmly confident for a reason. I realized at the surgery event and through to the 3 week post-op checkin the excellence in their personal care (bed side manner). The office and Dr. Gallagher communicated very well. Everything went smoothly and as planned. As nervous as I was about an unnecessary surgery, Dr. Gallagher and her team were able to build enough trust that I was able to receive a very beneficial result.

Dr. Gallagher and her team are the absolute best! She completed my brachioplasty on Tuesday and when I saw the initial results at my post op appointment two days later I literally had tears of joy streaming down my face. My weight loss journey has been a road of many ups and downs, and I was so nervous about whether this surgery could even work for me. Even with the swelling that is normal after brachioplasty, there is an enormous difference, and I am still in disbelief that even this early on, my arms look this great. My husband was even in disbelief, and we’re so excited to see what I’ll look like in two weeks time. Dr. Gallagher is so kind and patient, and walked me through all of the phases of healing/expectations, and was great to call and check in. If you are looking for a highly skilled surgeon that will give you optimal results along with a compassionate approach, it is well worth your time and money to consider a consultation with her. I am forever thankful for her and for her team! This is life changing for me. Thank you, Dr. Gallagher! I appreciate you so much!
