We welcome patients who have had their original surgeries elsewhere. There is a range of problems we can help patients with to align the appearance of their chest better with their transition goals.

Many of our patients come to Gallagher Plastic Surgery because their first surgery left them with scars, uneven contours, misplaced nipples, or excess tissue that affects how they feel about their chest. Dr. Gallagher in Miami, FL, works closely with each patient to evaluate their unique situation, explain the options, and create a revision plan that addresses both aesthetic and functional concerns.

What are Top Surgery Revisions?

  • Unfortunately, the greatest predictor of how a patient is going to heal in the future is how they have healed in the past. However, we can often get improvements by removing scars – especially in the case of “train-track scars”.

  • This can be a challenging issue to address, but in some patients, we can relocate one or both nipples if necessary. In cases where the nipple is way off, we can take it off and graft it back on in a more appropriate place.

  • It is possible to masculinize the nipple.

  • Nipples aren’t for everyone, and they can be easily removed in exchange for a short scar.

  • Patients are advised to wait at least six months, as the appearance of the chest will continue to improve after surgery. (This is especially true in key-hole procedures). But removing a little or a lot of extra skin is usually straightforward.

  • In some patients, a “sunken in” appearance is observed on one or both sides of the chest. It is sometimes possible to fill in one or both sides with the patient’s own fat to correct this.

  • In some patients, breast tissue may extend around the sides and back, leaving lumps behind after the first surgery. These are usually easily removed.

  • This can be a common problem when the patient has had a breast reduction at the first surgery, instead of top surgery. This is usually easily converted into masculinizing top surgery. Sometimes we see patients who have had keyhole surgery but still have too much tissue left. Sometimes a patient does well with a small-incision revision; otherwise, we can convert to a double-incision approach for a better appearance.

Who Are the Best Candidates For Top Surgery Revisions?

The best candidates are patients who have already had top surgery but are not satisfied with the results. Concerns may include leftover tissue, scarring, skin looseness, nipple size, or nipple placement. Often, we are able to help patients who have been told elsewhere that nothing can be done.

It is generally recommended to wait at least 6 months after the original procedure before considering a revision, to allow the chest to heal fully.

What to Expect on the Day of Top Surgery Revisions Surgery

On the day of surgery, you will check in at the surgical center and review your plan with Dr. Gallagher prior to the procedure. The length of surgery depends on the type of corrections being made, but most revisions are performed on an outpatient basis.

Anesthesia and numbing techniques are used to keep you comfortable.

After surgery, a supportive binder or garment will be placed, and you will be given detailed instructions for recovery. Having a trusted person to bring you home and stay with you for the first night is strongly recommended.

Your Guide to Post-Top Surgery Revision Care

Recovery after a revision depends on the complexity of the surgery, but is often similar to the initial recovery. Most of our top surgery revision patients will need to wear a binder or compression garment for a few weeks to help minimize swelling and promote healing.

Stitches are typically dissolvable, and most people feel well enough to return to work within one to two weeks. Some bruising, swelling, and temporary changes in sensation are normal and typically improve gradually.

Dr. Gallagher will provide you with clear aftercare instructions and schedule some regular follow-ups to check your recovery progress.

    Dr. Gallagher and her team are the absolute best! She completed my brachioplasty on Tuesday and when I saw the initial results at my post op appointment two days later I literally had tears of joy streaming down my face.
