What is Non-Flat Top Surgery?

An example of this would be a radical reduction in the breast size, but not removing as much breast tissue as in a typical masculinizing procedure, so that the patient still has some breast mound. The same thing goes for the size and shape of the nipples – we do not have to stick to the typical “male” nipple dimensions.

Non-flat top surgery can be performed with the same scars as regular double incision top surgery – usually designed to be placed underneath the pectoralis major muscle; however, we leave behind “mounds” that can vary in size according to the patient’s preference. This can be done with or without the nipple.

According to the patient’s preference, we can do a breast reduction – designing a smaller but still “feminine” shaped breast. Genderfluid patients may opt for this to facilitate easier binding. This is usually done with an anchor-shaped scar.

What to Expect on the Day of Non-Flat Top Surgery

Once anesthesia is administered and you are comfortably asleep, the surgery begins. For many patients, the incisions are made in the same locations as a double-incision top surgery, typically along the lower border of the pectoral muscles (this is customizable). Instead of removing all breast tissue to create a completely flat chest, Dr. Gallagher carefully reshapes and reduces the breast tissue, leaving behind mounds that match the patient’s preferences. The size and contour of these mounds can vary depending on your desired look, and nipple adjustments may also be performed to refine their size, placement, or shape.

In some cases, an anchor-shaped incision is used to achieve a smaller breast mound or a more feminine contour. This isn’t always necessary, however, and many patients will just opt to have the usual top surgery scar under the breast. Once the tissue has been reshaped and the incisions are closed, a supportive garment is placed over the chest to help reduce swelling and promote proper healing. The entire procedure generally takes between two and four hours.

After your surgery, you will wake up in recovery, where the team will monitor you closely before you are cleared to go home later that day. A responsible adult should accompany you to drive you home and stay with you for the first 24 hours.

Your Guide to Post-Non-Flat Top Surgery Care

Recovery varies depending on the extent of tissue reduction and reshaping; however, most patients experience mild to moderate swelling and bruising in the first couple of weeks following surgery. A binder or compression garment should be worn as instructed to help the chest heal properly. Dissolvable stitches eliminate the need for removal.

Most of our patients can return to work within 1 to 2 weeks, while strenuous activity and upper-body exercise should be avoided for approximately 6 weeks. Over time, swelling diminishes, scars soften, and the final chest contour becomes more apparent.

    Dr. Gallagher and her team are the absolute best!
