Introduction to Metoidioplasty

Metoidioplasty, often called “meta,” is a gender-affirming bottom surgery that creates a penis from the patient’s existing genital tissue. This procedure uses the natural growth of the clitoris that has occurred under testosterone, allowing Dr. Gallagher to craft results that look and feel authentic while aligning closely with each patient’s goals.

What is Metoidioplasty?

Currently, Dr. Gallagher does not offer urethral lengthening. This is a procedure where we use the patient’s clitoris, which has been enlarged after the effects of testosterone. This can be made into a small penis, and the labia are designed to form a scrotum. At a later surgery, testicular implants may be placed.

Usually, the penis made from the clitoris is not big enough to be used for penetration. The surgery is best done when the full effects of testosterone are seen on the clitoris – this usually means two years of testosterone before surgery. Often, this procedure can be combined with a monspubis lift, which brings the penis closer to the front of the body. It can be done with or without the creation of a scrotum and placement of testicular implants.

What Are the Benefits of a Metoidioplasty?

The main benefit of metoidioplasty is the creation of a penis that is natural, sensate, and entirely your own tissue. Because the clitoris has already grown under the effects of testosterone, the procedure allows for preservation of sensation and sexual function.

For many patients, metoidioplasty creates a more congruent body image, reduces or eliminates the need for external prosthetics, and helps align physical appearance with gender identity. Additional options, such as a mons lift or later placement of testicular implants, can further enhance results and create a more balanced contour.

Who Are the Best Candidates For Metoidioplasty?

The best candidates for metoidioplasty are transgender men or nonbinary individuals who are seeking gender-affirming genital surgery but do not wish to undergo phalloplasty or urethral lengthening.

Ideal candidates have been on testosterone long enough (usually at least two years) to achieve maximum clitoral growth, are in good overall health, and have realistic expectations about the outcomes. The procedure works better on patients with a BMI of under 35. This procedure is not typically used for penetrative sex, so patients should consider their goals carefully during consultation.

What to Expect on the Day of Metoidioplasty Surgery

Once anesthesia is administered, the procedure begins with the release of the clitoral tissue to create a penis. Depending on your goals, the labia can be reshaped into a scrotum, with or without testicular implants.

A Mons lift may also be performed to bring the penis forward for improved appearance and function.

Once complete, you will be placed in recovery for monitoring before being discharged home the same day. You should have someone available to drive you home and stay with you overnight.

Your Guide to Post-Metoidioplasty Care

Usually, this procedure takes 2-3 hrs and can be performed as an outpatient. Since we are not performing urethral lengthening, a catheter is usually not required for recovery. Most of our patients return to work in about 2 weeks.

    Dr. Gallagher and her team are the absolute best! She completed my brachioplasty on Tuesday and when I saw the initial results at my post op appointment two days later I literally had tears of joy streaming down my face.
